  • You can customize this template for use in your clinic to track employee hours by grant.

  • Simply customize the "Time Distr by Fund" table in the lower left corner of the time sheet to reflect your clinic's grants.

  • To adjust the dates for each pay period, enter the Start Date for the pay period at the top of the time sheet (in yellow) and the spreadsheet will calculate the dates automatically.

  • PLEASE NOTE: You may need to adjust the beginning and end dates to your clinic's work period.  This sample assumes a Monday through Friday work period.
