“Born in the 1960s out of American’s struggle for equality, social and ecomonic justice, health centers have always worked to improve not only the health and well-being of underserved people, but also to empower people to become actively involved in solving issues unique to their needs and communities. Increasing equity and reducing health disparities are daily practices.”
Health Centers are community-based and patient directed organizations that deliver comprehensive, culturally competent, high-quality primary health care services to the natioin’s most vulnerbale individuals and families. Health centers integrate access to pharmacy, mental and behavioral health, vision services, substance use disorder, oral health services, and other social services in areas where economic, geographic, or cultural barriers limit access to affordable health care. Ultimately Community Health Centers are integral in reducing health disparities by emphasizing coordinated care mangement of patients with multiple health care needs and the use of key quality improvement practices, including health information technology.
Community Health Centers can have federal designations.
- Federally Qualified Health Centers (FQHC) are community-based health care providers that receive funds from the Health Resources and Services Administration (HRSA) Health Center Program to provide primary care services in underserved areas. They must meet a set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes patients. FQHCs may be Community Health Centers, Migrant Health Centers, Health Care for the Homeless, and Health Centers for Residents of Public Housing.
- FQHC look alikes are health centers that like FQHCs improve the health of the nation’s underserved communities and vulnerable populations by expanding access to comprenhensive, culturally competnent, quality primary health care services. FQHC
